Privacy Notice and GDPR
To show that I am GDPR compliant and in line with the requirement of ICO, I set out the details below, clearly stating how all information about you will be used.
Brief notes are taken at the end of each session. These are recorded manually and stored in a locked filing cabinet. The only person holding access to the filing cabinet is myself, Sandra Mckeever. All notes are anonymous and no identifying names or personal details are included. The notes are an aide memoire and carry minimal detail. Each client/supervisee is identified by a code only. These notes are stored for three years after completion of our work together, when they will be shredded and destroyed. Three years is in line with the requirements of my Professional Body, the BACP and relates to any claims of professional misconduct.
Contact information: the information taken at our first meeting which includes contact details, name, address telephone number and information on doctor and health is stored separately from the notes above. This information is needed to identify you should I need to contact your doctor on your behalf. This would only be in situations where I have either your consent, or where I have serious concern about your mental health. This is also kept in a locked cabinet, of which only I have the key. The contract you signed is also kept with this information. This information is also stored for three years and then shredded and destroyed.
The information above is not shared with any other party, except when demanded by a court of law, where there is an issue of public safety (terrorism, drug trafficking or money laundering) or there is significant risk to another or self. As a Senior Accredited member of the BACP I have to attend supervision to monitor my work and my fitness to practice – anonymity is maintained and my supervisor is also data compliant. All information is otherwise held confidentially and no details shared. Should the client/supervisee request that information be shared – e.g. with a doctor or solicitor – this would be discussed in detail and a consent form would need to be signed should it be agreed that the information be released.
Clients and supervises are able to request to see their records at any time but four weeks notice is required. You are entitled to ask for any information I hold to be eliminated or corrected. The only information held electronically is in the form of emails and telephone numbers. Any emails will come through to my mobile. I will also hold your telephone number on my mobile in case I need to contact you in an emergency, but your number will be identified with your initials only. Both mobile and computers are password protected. Please be aware of the information you share within emails – it is my intention to limit information shared in emails to appointment times only.
